CARE PLAN PROGRAM

A’ la Carte CARE Services

Administration of medication- Nurse administration of medications, medication reminders, monitor, and/or special assistance. Verbal reminders for meals, activities of daily living and social activities

Assistance with Bathing- This service includes standby assistance during a bath, as well as physical assistance with bathing- getting into and out of the tub or shower.  Baths are given 3X week on a scheduled basis. 

Dressing/Undressing- This service includes standby assist when the resident performs the task as well as physical assistance from the staff to dress/undress.  In addition, AM/PM-Grooming Care- This service includes assistance with daily grooming and personal hygiene needs.  It includes physical assistance with all grooming tasks.

Ambulation- This service includes verbal reminders for meals or activities.  It also includes standby assistance from the staff while resident ambulates on their own as well as physical assistance to ambulate from one place to another whether inside or outside their apartment.  This service also includes getting those in wheelchairs who cannot propel themselves from one place to another by pushing their chair whether inside or outside their apartment.

Verbal reminders for residents that are NOT on Medication Administration for meals, medication, activities of daily living and social activities.

Toileting during the day- This service includes any assistance needed to cleanse after toileting as well as assistance needed to apply or remove any type of brief. This service is for those who are continent.  (Note:  This service does not include providing incontinence care -cleansing after an incontinent accident, proper disposal of a soiled brief or pad, as well as washing of linens or clothing as a result of an incontinent episode.)

Night-time checks- This service consists of staff entering the resident’s apartment at 10 p.m., 12 midnight, 2 a.m., and 4 a.m.   Staff checks resident to be certain the resident is safe and is in no acute distress.                                                                           

Night-time checks transferring- This service is provided to only those residents receiving night-time check service.  This service includes standby assistance while the resident transfers themselves as well as some physical assistance from the staff to and from bed, wheelchair, chair, or commode.

Night-time check toileting- This service is provided to only those residents receiving night-time check service.  It includes any assistance needed to cleanse after toileting as well as assistance needed to apply or remove any type of brief.  (Note:  This service does not include providing incontinence care -cleansing after an incontinent accident, proper disposal of a soiled brief or pad, as well as washing of linens or clothing as a result of an incontinent episode.)
